- Payment for your appointment
- You may also bring any medical records or have your doctor fax them to us at 719-228-9061.
- Referrals – If you are with an insurance company that requires referrals to our office, it is YOUR responsibility to make sure a referral has been processed for your visit.
- COST: We collect specialist co-pays, deductibles, and co-insurance at time of service please come prepared to pay. We accept VISA, Master Card, Discover and cash. We ask that you pay by card whenever possible. (Deductible and co-insurance amounts are based on your insurance plan.) You will be rescheduled if you do not come prepared to pay. ALWAYS ASK FOR A RECEIPT AND KEEP THIS FOR YOUR RECORDS!
IMPORTANT NOTICES
Insurance Cards & Insurance Changes- We need to see your insurance card at every visit. The card contains valuable information, which is required by your insurance company, to file the claim. We require all patients to notify us of any insurance change before their next appointment. This includes changes in co-pays, adding secondary insurance, and carrier or plan changes impacting deductibles. If we do not receive all your insurance information, it will be necessary to delay your appointment or bill you for the services.
Co-Pay, Deductibles and Co-Insurance – Many insurance companies require that you pay a co-pay, deductible or co-insurance for each office visit. That also applies to office visits with a specialist. Be prepared to pay your owing amount prior to seeing the physician. We are required by your insurance company to collect this. You will be rescheduled if you do not come prepared to pay. We accept cash, Visa, Master Card and Discover.
Patients accompanied by small children – The office is not a safe environment for small children. At the request of the providers, we will require you to have adequate childcare during the time of your appointment. If you come with your child, your appointment may be rescheduled.
Making Appointments – We ask patients schedule their next appointment prior to leaving. Our schedules fill quickly, and we do not offer same day availability.
Medication Refill: Our office requires appointments for all medication reviews, refills or requested changes. We do not manage medications over the phone or by fax. PLEASE PLAN ACCORDINGLY!

- A monthly appointment will be required for refills. In select circumstances, your provider may approve a prescription pickup without an appointment. The charge for this privilege is the patient’s responsibility. Your insurance carrier will not be billed. You can avoid this charge, by making your appointments prior to running out of medication. We will not increase medications over the phone for acute pain.
Procedures (Drivers) – If you are scheduled for a procedure that will be done under sedation it is mandatory you have a driver that will be able to remain in the office for the duration of your appointment. If your driver leaves, your procedure could be cancelled, and you may be charged a late cancellation fee of $145.00 prior to rescheduling. If this happens, sedation may not be offered for future services.
Pets – “Service” animals are allowed to accompany the patient, with identification vest. ” Companion/emotional support” animals will not be permitted.
APPOINTMENT CANCELLATIONS
New Patient Appointment Cancellation – Failure to cancel an appointment without proper advanced notice will result in a missed appointment fee. New patient office visits require 48-business hour notice, or a $180.00 Non-refundable missed appointment fee will apply. This amount must be paid in full prior to rescheduling. This fee will be applied to any out-of-pocket expenses for future appointments.
Established Patients – Failure to inform us of a cancellation without proper notice will result in a missed appointment fee. Office visit cancellations or reschedules require 24-hour notice, or an $85 fee will be added to your account. Missed appointment charges must be paid before rescheduling any future appointments.
Procedure cancellation – Cancellations for procedures done in the office or at the Surgery Center require a 48-hour notice or a $145.00 cancellation fee will apply. This amount must be paid in full prior to rescheduling.
